You have your Navigator call
A 60-minute phone call with your dedicated Care Navigator. We listen first. Then we help you identify the most urgent priorities, answer the questions that have been keeping you up at night, and begin building a clear picture of what comes next.
You receive your care roadmap
Within 48 hours of your Navigator call, you receive a personalized care roadmap. A clear, staged action plan built around your family's exact situation. Not a generic checklist. A document built for you.
Ongoing Navigator support begins
Your Navigator stays with you. As your situation evolves, new decisions arise, and new challenges emerge, you have a steady, knowledgeable guide in your corner. Someone who already knows your family's story and does not need to be caught up.
